CES Waiver The Division of Developmental Disabilities Services Waiver Services section provides a variety of community-based services to eligible individuals in addition to the management, policy development, planning and administration of the Medicaid Home and Community Based Waiver. The Community and Employment Support (CES) Waiver offers services in your community to support clients with intellectual or developmental disabilities (IDD) with all major life activities, such as living independently and working at a job with help from an employment coach. The program promotes inclusion for clients through community experiences and provides care coordination for getting services. The goal of the waiver is simple: it helps clients with IDD to live in the community rather than in an institutional setting such as a nursing home or Intermediate Care Facility.

About Us

Our goal is to provide care aides for those with disabilities who will help to encourage development and provide clients with independence and pride in their abilities. 

Our Executive Director 

Erica N. Taylor, MSNc, APRN, PMHNP-BC, is a highly accomplished and
compassionate Psychiatric Mental Health Nurse Practitioner with extensive experience
in both pediatric and adult behavioral healthcare. With a strong foundation in clinical
leadership, Erica excels in patient assessments, treatment planning, medication
management, and crisis intervention. Her ability to lead interdisciplinary teams, mentor
staff, and manage high-stress healthcare environments reflects her dedication to quality
care and professional excellence. She currently serves in leadership roles at The
Blossoms Nursing and Rehab and Mindful RX Psychiatric Office, where she provides
direct patient care while also overseeing clinic operations, staff supervision, and quality
assurance initiatives.

In addition to her clinical roles, Erica brings a wealth of experience in academic
instruction and mental health advocacy. As a former faculty member at Northern
Kentucky University, she contributed to nursing education through curriculum
development, clinical supervision, and student mentorship at the graduate level. Her
work at institutions like Pinnacle Pointe Hospital and Chenal Family Therapy further
highlights her expertise in psychiatric evaluations, telehealth, and evidence-based
medication management. Erica’s skill set is complemented by her strong organizational
abilities, commitment to patient-centered care, and passion for advancing the field of
psychiatric nursing through education, advocacy, and leadership.
